No one can eliminate the void, shock or grief that people experience following the death of a loved one. However we can eliminate some of the strain by helping you file a claim.

Step 1 |
Notification |
|
|
Report the death to John Hancock (U.S.A.) by either:
a) Using our quick and easy Notification Form OR
b) Contacting John Hancock (U.S.A.) at 1-800-387-2747 and asking for a Claims Analyst
Following this notification we will send a claim form and a letter outlining our claims requirements.

Completing the Claim Form |

The designated beneficiary must provide:
a) A certified copy of the death certificate
b) The insured’s policy(ies) AND
c) A completed claim form
|
|
|
Print either the standard Death Claim Form - PS5119US, or if death claims relates to a New York resident print the Death Claim Form PS5150(NY) or if the beneficiary resides in one of the following states, Alaska, Arkansas, Arizona, California, Colorado, Florida, Kansas, Louisiana, Maryland, North Carolina, North Dakota, Nevada, or Vermont, print the Death Claim Form - PS5120US, and send or personally deliver them to the beneficiary.

Do not complete a claim form if it is the first death of a survivorship policy (only Notification is required). |

|
|
|
NOTE: There may be other requirements depending on various circumstances (these will be outlined in the letter you receive after notification). |

Submitting Requirements |

|
Submit all requirements to the Claims Department. Please refer back to the Claims Overview tab for address information. |

Step 4 |
Receiving Proceeds |
|
|
The death benefit will be disbursed via check, Safe Access Account (not available for John Hancock New York), or electronic funds transfer (depending on the amount and your preference). The disbursement will include interest. |
